We take this responsibility very seriously and it is essential that we have accurate information about our employees’ medical conditions in order to assess their fitness for work.

We would be grateful if you could provide us with information about [Employee Name]’s medical condition, any limitations or restrictions on their ability to carry out their job, and any adjustments that may be required to support them in the workplace.

Please also let us know if there are any likely implications for their health and safety or that of their colleagues. We assure you that any information provided will be treated with the utmost confidentiality and used only for the purpose of assessing fitness for work.
